Application purpose

The patient application is intended to support authorized participation in PHX services. It does not replace a clinician, emergency service, or complete medical record.

Account responsibility

Users will be expected to protect access credentials, provide accurate information, report compromise, and use consent and representative functions lawfully.

Clinical safety

Users must seek qualified care for medical decisions and use local emergency pathways when urgent help is required.
